Please download fill out and bring the health intake form to your first appointment. If you can't fill out the form in advance, please arrive 10 minutes earlier to fill it out in our clinic before your first treatment.

24 hours advanced notice for reschedule or cancel
appointments
When we reserve an appointment for you, there are three people involved: you, the therapist, and another patient waiting for an open appointment. We require 24 hours notice should you need to reschedule or cancel your appointment, otherwise a missed appointment fee of the full amount will be billed.
